Mission/Vision/ Philosophy

At Jade Health and Wellness, we offer both group and individual therapy services. We offer medication management and Medication-assisted treatment services. The types of therapies utilized in the clinic are Cognitive behavioral therapy, Talk therapy, the Matrix model/ Life Skills for IOP groups, and trauma-informed care. Jade Health and Wellness provides services to clients 12 and older. Jade Health and Wellness prides itself on providing individualized care based on individual needs. Jade Health and Wellness believes that when trust is reciprocated with the client and staff better outcomes are achieved. Jade Health and Wellness encourages family involvement to increase resources for clients and to increase success for clients.

What sets Jade Health and Wellness apart from other clinics is that from the first time a client interacts with staff. Staff set out to understand the client and use current and past concerns to create an individualized care plan for the client to make sure they are successful. We pride ourselves on monitoring Client to client-to-staff ratio so that the client is always first their needs will be met and most importantly the client feels heard.

Many adults who develop mental health disorders also struggle with substance use and addiction. If co-occurring addiction is not identified and addressed during treatment, this can have a significant negative impact on an individual’s ability to make sustained progress toward improved health.

Behavior Health services

At Jade Health and Wellness in both our outpatient and IOP programs we provide a variety of therapeutic interventions, programming options in our co-occurring disorders unit also include medication-assisted treatment (MAT), and 12-step education and support.

As is the case with all programming at Jade, Health and Wellness clients who receive care in our IOP program will follow personalized plans that are based on a thorough assessment of their unique strengths, needs, and goals.

Treatment

Medication management – Everyone receiving care will complete a psychiatric assessment to assess the need for medication management to help the client while receiving services to remain stable and have better success in the program. Not everyone will be prescribed medications, but those who will have the opportunity to meet with a Licensed Independent Practitioner, and or a nurse practitioner on a weekly and as-needed basis to ensure that the medications are appropriately alleviating their symptoms and to make any changes that may be necessary. Clients suffering from substance abuse will also have the option to receive medications to decrease relapse and provide stability for the client while in the program.

MAT services
Clients are provided with policies and procedures for opiate treatment. Clients are educated on treatment for opioid use disorder and services not provided by the clinic are referred to other providers who provide services specific to the client.

Clients are also given the option to see a primary care physician either in-house or referred to other providers for health screening. Before MAT services start providers assess the client for the appropriateness of medications. Providers utilize the Az PMP Aware website for each client to decrease diversion and doctor shopping. Clients must sign policies and procedures for MAT services which include the process of medication being started, labs, urine drug screening, and random drug screen testing. Providers and facilities utilize and adhere to policies and procedures maintained by SAMHSA.

Individual therapy – Individual therapy sessions are held weekly and on an as-needed basis for clients receiving treatment. Clients will also meet with a social worker or other licensed practitioner to process their experiences in treatment and to address any problems or concerns that may arise.

Group therapy – Group therapy sessions help clients to express themselves among peers who may be dealing with challenges that are like their own. This is a great setting for individuals to give and receive support, and to understand that this issue is not just there’s alone, there are other individuals who suffer from the same issues. Group therapy sessions are a source for patients as they progress through treatment.

Telehealth Services
Our IOP and outpatient program currently offers virtual programming for those who are unable to attend in-person group therapy and individual therapy services. This clinician-led option meets Mondays, Wednesdays, and Fridays from 10 a.m. to noon and 1 p.m. to 3 p.m. and gives patients a chance to interact exclusively with their virtual group while receiving the same benefits of treatment they would experience face to face.

Discharge Services
Jade Health team also prides itself on being aware of services in the community. Where staff/case managers can help clients with services such as various housing programs here in Arizona and if a higher level of care is needed inpatient and residential services

Client discharge planning starts from the first day they enter the program and the whole interdisciplinary team is a part of the discharge planning process, so the client feels supported to help maintain their sobriety and stability.

The discharge plan supports the individual needs by going over the treatment and changes made to the treatment plan. Skills and needs from when the client first entered the program to the last day will be assessed when the client is discharged. Clients will be given referrals and walked through the process for various programs the client and the team feel fit to help the client in their process. Clients will be allowed to continue to come to the clinic for 12 steps and SMART Recovery classes and they will be given other resources.

Continuum care will be included in the client’s discharge plan whether this be staying with the therapist and providers in the outpatient program or other facilities. The client will continuously be educated about relapse, the prevention of relapse, and resources for relapse. Crisis planning starts from the admission process to the discharge. Staff continuously engage support systems listed by clients whether this be families or communities in the admission and discharge process.

Inclusion/Exclusion criteria
Our intensive outpatient program (IOP) is focused on adolescents and adults ages 12 and older who suffer from mental health and substance abuse issues. Clients are placed in age-appropriate groups. young adult women struggling with depression and anxiety. The IOP is a helpful alternative to inpatient psychiatric admission and can be a crucial tool for those recently discharged from inpatient or residential settings. The IOP is run by independently licensed providers and staff from various backgrounds who have experience in peer support, weblebrity, 12-step, and SMART Recovery who work closely with referring and non-referring providers to support their patients.

Eligibility
Adolescents and adults ages 12 and older who are dealing with symptoms of both mental health and substance abuse issues that interfere with their day-to-day functioning.

We provide care for:

Depression
Anxiety disorders
Bipolar Disorder
PTSD
Age-related psychological challenges
Opioid Use
Alcohol use
Methamphetamine use
Our IOP is not geared toward individuals who are in imminent danger to themselves or others.

Services
The IOP group meets for 12 weeks on Mondays, Wednesdays, and Fridays from 10 a.m.-12 p.m. and then 1 p.m. to 3 p.m. Tuesdays and Thursdays Weblebrity classes, Yoga, Arts, and skill classes are offered from 10 am to 1 pm

The sessions are run by BHTS under the supervision of Independently licensed providers and therapist

Therapeutic modalities include cognitive behavioral therapy (CBT), mindfulness, motivational interviewing, expressive arts, and more.

All sessions are held in person and online
